Book your initial visit online or call Sage & Vine Aesthetics’s front desk at (804) 517-5168. You’ll receive two forms to complete: a medical intake form and a consent form.
You will need to complete your intake paperwork prior to your first appointment. This is sent to you via email after you scheduled your first treatment. Once completed, you will not need to fill this out again for another year. Print or email a copy of your blood panel results to hello@sagevineaesthetics.com. All medical files are transferred to our HIPAA compliant system.
